Form - CIPPE Section 6-7 Physical Forms.pdf**Section 6 Health History Form - To be completed by the parent/guardian in its entirety and presented to the Authorized Medical Examiner performing the physical for review. **Section 7 Preparticipation Physical Evaluation - To be filled out by the Authorized Medical Examiner and returned to the athletic office prior to the first practice or contest as part of the registration process.
