PHYSICAL FORMS - PIAA Section 5 & Section 6

**Section 5 Health History Form - To be completed by the parent/guardian in its entirety and presented to the Authorized Medical Examiner performing the physical for review. **Section 6 Preparticipation Physical Evaluation - To be filled out by the Authorized Medical Examiner and returned to the athletic office prior to the first practice or contest as part of the registration process.

*If your child was injured during the season or between seasons, you must submit this form in order for your child to be cleared to participate.
